Baseball Fee Information
- A $25 annual family membership fee will be charged for new members and for returning members who did not pay the fee during the 2010 registration periods. Please contact Dennis Primavera at drprimavera@aol.com if you believe that an annual family membership fee was incorrectly charged.
- The family membership fee is not refundable. If a player withdraws, a $5 administration fee will be imposed. No refunds after April 18.
Certificate of Registration Information
- After completing the Player Registration Form including online payment information, click the "submit" button one time only. Please be patient. It may take a while for your information to be processed.
- If for any reason your payment is not approved, or you are kicked off the browser, please re-register.
- All registration forms from this page are on secure sites.
- Once the form is processed, you will receive a Certificate of Registration.

NOTE: If you have played Baseball, Girls Lacrosse, Soccer, Indoor Soccer, Basketball during 2009 with CHYSC, please choose "Returning player" for the desired sport on the list below. If you have not played a sport with CHYSC during 2009, please choose "New player" for the desired sport on the list below. Proof of age is required for all new players.
