PLEASE DO NOT PRINT OFF THE REGISTRATION FORM. COMPLETE THE ONLINE REGISTRATION. IF YOU ARE UNSURE AS TO WHETHER IT COMPLETED OR NOT, THEN PLEASE SEND AN EMAIL TO THE OFFICE FOR VERIFICATION AND WE WILL GLADLY VERIFY THE REGISTRATION OF YOUR CHILD OR CHILDREN.

IMPORTANT INFORMATION:

• Registrations are not considered complete until all monies and required documents, such as birth certificates (if needed) are received.

• Be careful using a scroll mouse when making selections on the application form as it may alter the selections made.

• A late fee of $25 will be charged for players registering after the deadline date of Jan 31 or June 30.

• MYSL Refund Policy: There will be no refunds for any reason other than the following:
  • Not being able to place a child on a team roster as a result of full divisions
  • A lack of coaches
  • Players medically unfit to play the first scheduled game (documented by physician's statement)

• A COPY OF THE PLAYER'S BIRTH CERTIFICATE MUST BE SUBMITTED FOR ALL NEW PLAYERS TO MYSL. You will receive the appropriate instructions during your registration, if applicable. Please note that a new registrant's registration will not be complete for team assignment until the new player's birth certificate has been received per instructions provided.

In order to successfully submit your registration information you must:

1)	Complete all requested information for each player
2)	Complete the payment option (Registration is not complete w/o making a payment option).
3)	Print the Certificate of Registration Page
