| For completion by adult participants of WVSA Member Clubs whether coach, assistant coach, substitute coach, staff coach, skills trainer, coaching coordinator, athletic trainer, program administrator, referee, referee assistants, fourth official, assessor, instructors, referee coordinator, ODP coaches and administrators, and Club officer employed, serving as a volunteer of, serving as an independent contractor of, or endorsed by a Club or a team of a Club (each such person being 'associated' and having an 'association' with such Club) |
| The WVSA Risk Management program is designed to foster safe circumstances for every person, and especially every child, who participates in a US Youth soccer affiliated activity. It is a program to inform all WVSA personnel and WVSA members concerning the risks associated with our youth soccer programs. This form will be kept in strict confidentiality by WVSA or each of its respective members and will be updated annually. |

If you have already done so, and have additional positions (another club, or another position in your club that you forgot to list on the first application, or both) that you must add to your submitted application, please select 'I want to add another volunteer position'. |
