LOCATION:
All games will be played at Jupiter Community Park, Jupiter Lighthouse Park and Jupiter Farms Park.
 
DOCUMENTS NEEDED:
Documents needed for participation: Due at Friday night team check in, four copies of roster, copies of notarized medical release forms and guest player(s) disclosed.
 


In order to complete your registration, please complete the following:

1) Complete all required team information.
1) Complete online payment.
2) Print the Certificate of Registration.
3) Mail the required Enclosures, the Certificate of Registration, and the check if you have not paid online to the address stated on the Certificate of Registration.
