IMPORTANT INFORMATION:

• Registrations are not considered complete until all monies and required documents, such as birth certificates (if needed) are received.

• Be careful using a scroll mouse when making selections on the application form as it may alter the selections made.

Refund Policy:
It is the policy of the Chesterfield Strikers that no refunds (full or partial) shall be given except under the following circumstances:
- In the event of documented injury/illness which prevents a player from participating in the program for which that are currently enrolled/registered.
- Player moves more than 50 miles outside the Richmond Metro area
- Chesterfield Strikers is unable to place a player on a team

In the above instances, the following applies:
- Written notification received prior to first scheduled practice – full refund
- Written notification received prior to third scheduled game – 50% refund
- There will be no refunds given after the third scheduled game.

A $25 administrative fee will be assessed for all refunds.

In order to successfully submit your registration information you must:

1)	Complete all requested information for each player
2)	Complete the payment option
3)	Print the Certificate of Registration Page
